The No Surprises Act protects patients from unexpected medical bills. The goal of the No Surprises Act is to protect patients from unexpected medical costs and to ensure that they can access the care they need.
Questionnaires/Screening Tools
If a clinic staff member directs you to send a completed form back to us, please scan (or take a clear picture with your phone) and email the form back to forms@tccob.com. If you are sending back identifiable health information, you are advised to have us send you a secure email first to which you can attach the forms and send back securely.
